1. 报名资料均完整，方受理报名。
2. 收到报名资料后，预计于活动前 14 天发出行前通知。
3. 若超过上述预计时间仍未收到通知，请主动致电主办单位查询：法鼓山 03-7490 2298 / 03-7490 2299。服务时间: 9:30am～6:00pm。
4. 因故退出皈依典礼者，敬请提早通知，恕不接受转让名额予他人。
5. 如有任何讯息布达，法鼓山将以电邮（[email protected]）通知。若没收到，请先检查您的电邮中的 spam 或 junk email。
6. 皈依典礼全程使用中文进行，若需要英文翻译，可在报名表格注明。
7. 请穿着有袖上衣及宽松长裤，以方便皈依行仪。
8. 皈依者需全程参与三皈五戒开示，及授皈依仪程，始算正式皈依。若未全程参与皈依仪式者，不发皈依证。
9. 皈依典礼之后，请到接待处领取皈依证。
10. 随喜供养三宝，护持建设道场，打斋供众。（方式：点击查看 ）

Important Notes: 

1. Only fully completed registration forms will be processed. 
2. A confirmation email will be sent 14 days before the event.

4. If you are unable to attend, please kindly inform us in advance. Please note that registered slots cannot be transferred to others. 
5. All updates will be sent via email ([email protected]). Please check your spam folder. 
6. The ceremony will be conducted in Mandarin. English translation is available upon request. 
7. Please wear loose-fitting clothing (long pants and T-shirt) for ease of participation. 
8. Participants must attend the full ceremony, including the Dharma talk on the Three Jewels and Five Precepts, to receive the Refuge Certificate. 
9. Certificates can be collected at the Reception Area after the ceremony. 
10. Offerings to the Three Jewels and support for the building fund are welcome.

注意事项：

1. 主办单位将在大出坡期间为义工购买保险。然而，鉴于保险公司的相关规定，70岁及以上的菩萨不在保险的承保范围内。接受并认同此规定者方可报名参加本活动。
2. 請把車子泊在道場外合适的位置。
3. 若您因故不克参加，敬请尽早告知专职 03-7490 2298 / 7490 2299。
